Hypotension is the most common complication in the operating theatre and PACU (1) – current standards of blood pressure measurement do not provide sufficient monitoring.
Hypotension and the oxygen deficiency connected with it is a common reason for perioperative complications and postoperative mortality:
- 56% of cardiac arrests during surgery are preceded by hypotension (2)
- Every single minute of hypotension raises 1-year postsurgical mortality risk by 3% (3)
For early diagnosis and counteracting hypotension efficiently, generally neither upper arm NBP nor intra-arterial measurement are recommended.
Upper arm NBP measurement is intermittent and in short intervals painful for the patient (4) and can therefore not avoid the risk of hypotension:
- 44% of hoyptensive episodes are missed or recognised too late (5)
- The duration of such episodes is thus mostly underestimated (5)
Intra-arterial measurement is not used in 82% of the cases (6):
- Painful for the patient
- Expensive, time-consuming and risky
- Only a high risk (ASA 4) and the need for additional blood values (e.g. blood gases) justify invasive measurement
